Once this form is submitted, you will receive an email requesting your resale certificate. Once we have that on file, we can confirm your trade membership is active and you will be given a discount code that can be applied to all of your transactions. 

Please note that all images, copy, and designs (collectively, "Materials") are the property of Otea Textiles. While you are granted the authority to reproduce images as is required to conduct business with your individual interior design and decorating clients, the use of these images by you, or anyone else authorized by you in any other capacity, is prohibited unless specifically permitted by Otea Textiles. Any unauthorized use of the Materials may violate copyright laws, trademark laws, the laws of privacy and publicity, and communications regulations and statutes. Your membership in the trade program should not be construed as granting, by implication or otherwise, any license or right to use any trademark, service mark, or other Materials without the written permission of Otea Textiles. Membership may be denied or terminated at any time, with or without cause, at Otea Textile's sole discretion.
